Guard the span start, not only the start_as_current_span(...) call.
The OpenTelemetry SDK defines Tracer.start_as_current_span as a lazy context manager (@_agnosticcontextmanager). Calling the method only builds a generator object. start_span, the sampler, and SpanProcessor.on_start run later, inside __enter__ at with span_context as span:. That with statement is outside guard_trace.
For an SDK tracer, the guard at Line 37 and Line 119 therefore never catches a span-start failure. span_context is never None, so the untraced fallback never runs. A span-start failure then propagates to the tool caller, and the tool never runs. This defeats the PR's stated isolation goal. test_tool_execute_runs_when_tracer_fails hides the gap because _BrokenTracer.start_as_current_span raises immediately instead of at __enter__.
The Agent and Team non-streaming wrappers in __init__.py have the same root cause: Lines 218-228, 380-390, 484-496, and 567-579. The streaming paths use start_span, which runs immediately, so they are not affected.
Fix: start the span eagerly inside the guard, then activate it with trace.use_span. Also update the fake tracer so that its failure happens at __enter__ time.
🐛 Proposed fix (sync tool wrapper; apply the same pattern to the other sites)

Guard span-context exit failures.
guard_trace does not cover with span_context as span:. If OpenTelemetry raises from context_api.detach(token) or span.end() during exit, that instrumentation exception can escape after a successful tool, Agent, or Team call. This is separate from eager span creation. Wrap the context-manager exit, not the wrapped call, so application exceptions still propagate unchanged.

Keep exception logger failures inside guard_trace.
When span creation raises, guard_trace calls Config.exception_logger(e) before the wrapper reaches its span_context is None fallback. If that callback raises, the callback exception escapes and the wrapped call does not run untraced. The callback must not break tracing-failure isolation.

Problem
With the agno instrumentor active, a failing tool or agent call is hidden from the caller. Six wrapper
__call__methods are decorated with@dont_throw, and agno's decorator swallowsExceptionfor both sync and async calls. The wrappers call the real SDK method and thenraiseon failure, so the decorator catches that re-raise and returnsNone:_tool_wrappers.py:23/:93—FunctionCall.execute/aexecute; a tool failure returnsNoneinstead of raising.__init__.py:149/:298/:450/:523—Agent.run,Agent.arun,Team.run,Team.arun; an agent or model failure returnsNone. The span itself may be marked ERROR, but the application continues as if the call succeeded. That is the situation this instrumentation exists to make visible, so returning a plausible-looking
Noneis worse than no tracing.Fix
Remove
@dont_throwfrom these wrappers and guard only the instrumentation blocks, using a newguard_tracecontext manager inutils.py— the same pattern as #4589. Each real call sits in its owntry/exceptthat records the exception on the span and re-raises. If the tracer cannot open a span, the real call runs untraced instead of failing because instrumentation is unavailable. The publicdont_throwhelper is kept for other callers.Test
The new file drives the real wrappers with a fake wrapped callable that raises, and asserts both that the exception reaches the caller and that the exported span is ERROR with an
exceptionevent (exception.type == "RuntimeError"). It covers sync and async tool execution,Agent.run,Team.arun, and a broken-tracer fallback. On the PR template's screenshot request: I cannot attach an observability-platform screenshot, so the evidence is the exported span itself, read back from the in-memory exporter and asserted rather than eyeballed.
Related, different path: #4589 fixed the same decorator pattern in the MCP instrumentor; this is the agno instrumentor.
中文说明
agno 插桩的 6 个 wrapper
__call__带@dont_throw。
公开的dont_throwhelper 保留。验证:base 94d3f04 上新测试 5 failed(
DID NOT RAISE),分支 5 passed、包内 15 passed、ruff check干净。新增测试用会抛异常的假 wrapped 调用真实 wrapper,断言异常传回调用方且导出的 span 为 ERROR 并带 exception 事件,覆盖同步/异步工具、Agent.run、Team.arun与 tracer 故障回退。Summary by CodeRabbit
