Record queued Codex message reads via prompt matching and receipt thread IDs

• ledger.take_prompted marks a session's queued messages as read when a prompt.before payload's text matches the message (trimmed), before the postbox hook's capability check in adapters.rs
• queue.queued_outcome records the queued ledger event under the thread ID parsed from Codex's receipt instead of the posted session name
• Non-UUID session strings now emit a Codex name-lookup warning, and queue.failure_outcome can extract a --session UUID hint from refusal stderr in mod.rs
• ledger.take is a new shared locked read-and-append helper; take_posted now uses it, and a folded Message now reports the session from its last event
• Includes design/spec docs, proof recordings for 4 scenarios, and tests in ledger_tests.rs, tests.rs, and queue.rs
• Behavioral Change: message session now comes from the last ledger event rather than the first (see fold in ledger.rs); queue posts with named sessions gain a warning in their output
