Exercise actual token login, authenticated usage, local file creation and remote coding completion. Repair the live auth test to call usage summary and verify the identity scope. Testing: make test passed; live login with source and published binaries passed; local dev file-creation test passed. Production cloud test currently reproduces the API 402 pending release.
Testing: full CLI suite, live token login, local dev file creation, and completed production cloud session. Risk: medium; --cloud now selects remote sessions and --metered selects local billed inference. How to revert: install v8.5.751.
Testing: full CLI suite rerun; first-match lookup still tested with two executable fixtures. Risk: low; production lookup delegates to the same pure path search. How to revert: revert this commit.
