fix: address review findings on workflow skill docs and agents models - #447
CodeRabbit's reviews on #440 and #441 arrived after they were merged. This addresses the findings that hold up against the code.
For the workflows skill, the resume section now states both refusals the manager enforces: RECOVERY_REQUIRED for any run whose calls used writeMode: "allowed", isolation, or workspace (read-only ones included), and RECOVERY_CONTEXT_CHANGED when the project context changed. The patterns page says up front that edit loops restart rather than resume. The limits note was too broad: running a script in the sandbox showed a stack overflow ends the run while heap exhaustion throws a catchable InternalError: out of memory, and it now says exactly that. The QA-gate example no longer applies a final fix it never tests, so its verdict always comes from a test run. A directory sitting where a supporting skill file belongs is skipped instead of failing every workspace open.
For devspace agents models: a built-in provider left out of an explicit providers list can still be listed by name, as the docs promise. providerErrorDetail now reads the agent's stderr from process-launch causes shaped { error, stderr }, so ACP initialization failures show their reason here and in agent runs. ACP session/new and Codex model/list are bounded at 30 seconds, so a hung harness reports a timeout for itself instead of stalling every provider. opencode models launches through cross-spawn like the server, so Windows .cmd shims work. Pi resolves the listed provider/id before a gateway model's bare id.
Verified with the full suite, new tests for the error-detail shape and the Pi collision, and against the real harnesses (agents models opencode still lists 129 models; agents models copilot now answers although copilot isn't in the config).
Named providers can use a built-in default when no driver is configured.
Model listings for ACP, Codex, and OpenCode now have time limits and report timeout errors. OpenCode listings also report errors when output exceeds the limit.
Bug Fixes
Model selection prefers an exact provider-and-model match when available.
Provider launch errors can include standard error details, with authorization values redacted.
Managed skill syncing skips directory targets other than SKILL.md.
The QA gate returns failure details on its final unsuccessful attempt without applying another fix.
Documentation
Clarified workflow resume restrictions, recovery errors, and run-ending versus catchable limits.
Workflow skill (#440):
- Resume docs now state the RECOVERY_REQUIRED refusal for runs whose
calls used allowed write mode, isolation, or workspace (read-only
ones included) and the RECOVERY_CONTEXT_CHANGED refusal.
- The limits note now says stack overflow ends the run but heap
exhaustion is a catchable InternalError, as verified in the sandbox.
- The QA-gate pattern no longer applies a final fix it never tests.
- A directory at a supporting skill file path is skipped instead of
failing every workspace open; SKILL.md stays strict.
agents models (#441):
- A built-in provider left out of config can still be listed by name.
- providerErrorDetail reads the agent's stderr from process-launch
causes ({ error, stderr }), which also fixes ACP init errors in runs.
- ACP session/new and Codex model/list are bounded at 30s so one hung
harness cannot stall the whole command.
- opencode models launches through cross-spawn, so Windows .cmd shims
work like the server launch.
- Pi resolves the listed provider/id before a gateway's bare model id.

Walkthrough
The changes update workflow recovery and QA guidance, provider selection and model matching, model-list request handling, provider error details, and managed skill synchronization.
Changes: Workflow recovery and QA
Layer / File(s)
Summary
Resume and run-limit guidance skills/workflows/patterns.md, skills/workflows/reference.md
The guidance limits resume to eligible read-only checkout runs and documents recovery refusal conditions. It distinguishes run-ending limits from catchable sandbox heap exhaustion.
Final QA attempt skills/workflows/patterns.md
The QA loop returns the final attempt’s failures without applying another fix. Earlier failed attempts still proceed to the fix step.
Provider and model handling
Layer / File(s)
Summary
Provider fallback and model matching src/cli.ts, src/local-agent-pi.ts, src/local-agent-pi.test.ts
Model lookup uses a built-in provider driver when no configured driver matches. Pi model resolution prioritizes a unique exact provider/id match, with a test for an ambiguous bare ID.
Bounded model listing src/local-agent-acp.ts, src/local-agent-codex.ts, src/local-agent-opencode-version.ts, src/local-agent-opencode-version.test.ts, src/local-agent-opencode.ts
ACP and Codex model-list requests use 30-second deadlines. OpenCode model listing uses runOpenCodeModels, which handles command output, errors, a 60-second timeout, and an output limit. The test checks process-tree termination after oversized output.
Provider error details
Layer / File(s)
Summary
Error details and redaction src/local-agent-events.ts, src/local-agent-events.test.ts
Error detail extraction returns redacted, bounded stderr when available. Otherwise, it continues through cause or error. Authorization values are fully redacted. Tests cover stderr and nested-error cases, plus Basic and Bearer values.
Managed skill synchronization
Layer / File(s)
Summary
Directory target handling src/skills.ts
Synchronization skips directory targets for supporting files. A directory target named SKILL.md still raises an error.

- Redact the whole Authorization value (any scheme), since agent stderr
now reaches error details and Basic credentials survived.
- Escalate opencode models from SIGTERM to SIGKILL and cap its output at
4 MiB, as the replaced execFile did.

Reject only once the child closes (or after the SIGKILL escalation),
and run it in its own process group so the stop reaches processes it
started. A real-process test covers a listing that ignores SIGTERM and
floods past the cap.

Guard stop so it runs once per listing.
186-187: Minor · Performance & Scalability · Low value
Guard stop so it runs once per listing.
After the byte cap is crossed, every later stdout or stderr chunk calls stop again. Each call sends another SIGTERM and creates another SIGKILL timer. The timers are bounded by the grace window, so the cost is small. A settled check at the top of stop avoids the repeated work.

Update the stop function used by the OpenCode model listing to return
immediately when the listing is already settled, preventing repeated termination
signals and kill timers; preserve the existing termination and finish behavior
for the first call.

On the follow-up to guard stop so it runs once: it already returns early when a stop is in progress or the call has settled (if (stopError || settled) return;).

Keep the SIGKILL escalation timer referenced so it runs before the CLI exits.
181-185: Minor · Stability & Availability · Quick win
Keep the SIGKILL escalation timer referenced so it runs before the CLI exits.
stop sends SIGTERM to the process group, then schedules the group SIGKILL in a timer that calls .unref(). The close handler rejects once the child closes, which can happen before that timer fires. In devspace agents models opencode, nothing else holds a referenced handle after that rejection, so the process can exit before SIGKILL runs. A same-group descendant that ignores SIGTERM and has its output redirected then survives.
Without a provider name, agents models waits on all enabled providers, so their pending work can keep the process alive. In that case the window depends on them.

Review comment at @src/local-agent-opencode-version.ts around lines 181 - 185:
Keep the SIGKILL escalation timer in the stop flow referenced by removing its
unref call, so it runs even if the child’s close handler rejects and no other
handles keep the CLI alive. Leave the later timer that calls finish(error)
unchanged.

The listing runs in its own process group, so a terminal interrupt did
not reach it. Forward SIGINT and SIGTERM to its stop path while it runs,
and keep the SIGKILL escalation timer referenced.
