Adds build_restart_initial_nodes(ages=...). At each given age it admits every living regime that is active then. The baseline and ACA model constructors and create_benchmark_model gain an initial_nodes argument.

The default remains the estimation sample's starts: entry ages 51–60 in the five pre-claiming, pre-Medicare regimes.

A simulation can now pick up from the states another simulation reached at some age. The 2014 ACA introduction ("MIT shock") uses this: each person's simulated 2014 state is continued under the baseline and under each ACA variant.

`build_restart_initial_nodes(ages=...)` admits every living regime active at
each given age, and both model constructors (and the benchmark) take
`initial_nodes`. The default stays the estimation sample's entry starts.
