fix(security): close eval fail-open paths and redact live identifiers - #233
Approval scorers took the first ACTION token so a later SEND could hide behind ASK and v1 treated "false" as approval. The dispatch gate considered any truthy revoked value other than True as active. The S3 guard accepted a partial public access block. The governance verdict defaulted prerequisite-result to success so run-quality false could skip quality and still pass. The published docs referred to live Make, Cloudflare and relay identifiers.
Scorers now require a line-start whole token and will treat any SEND as the action. Revoked must be a boolean. All four of the S3 public access flags must be boolean true. prerequisite-result defaults to empty and run-quality false only passes on exact success. The composite action will point to the main commit that contains it. Live scenario, worker and execution IDs are all stripped from the published docs.
Seeded Odin bootcamp defects remain as exercises.
Summary by CodeRabbit
Bug Fixes
Approval and delegation evaluations now recognize only standalone decision tokens, apply consistent rules when multiple decisions appear, and reject invalid approval metadata.
Infrastructure policy checks now require all public-access protections to be explicitly enabled.
Quality gates now require an explicitly successful prerequisite when external quality checks are skipped.
Documentation
Clarified prerequisite-result requirements and updated operational guidance; sensitive implementation details and identifiers are no longer published.
Chores
System-quality checks now run on pushes to the main branch only.
Approval scorers took the first ACTION token, so a later SEND could
hide behind ASK, and v1 treated the string "false" as approval. The
dispatch gate treated any truthy revoked value except True as active.
The S3 guard accepted a partial public-access block. The governance
verdict defaulted prerequisite-result to success, so run-quality false
could skip quality and still pass. Published docs named live Make,
Cloudflare, and relay identifiers.
Scorers now require a line-start whole token and treat any SEND as the
action. Revoked must be a boolean. All four S3 public-access flags must
be boolean true. prerequisite-result defaults to empty, and run-quality
false passes only on exact success. The composite action pin points at
the main commit that contains it. Live scenario, worker, and execution
identifiers are removed from published docs.
Seeded Odin bootcamp defects stay as exercises.

The pull request changes the governance prerequisite gate, evaluator parsing and validation, and S3 public-access policy checks. It also updates operational and specification documents, including references to the Kiro relay and Venture validation scenarios.
Changes: Governance prerequisite gate
Layer / File(s)
Summary
Prerequisite gate and validation .github/workflows/governance.yml, .github/workflows/system-quality.yml, tests/test_quality_workflow.py
The prerequisite input defaults to empty. The verdict combines prerequisite, quality, policy, and secret-scan status. The system-quality trigger and action reference change. Tests cover the gate rules.
Scorers parse action tokens at line starts, prioritize SEND, and otherwise select the last parsed action. The v1 scorer accepts approval metadata only when it is exactly True. Tests cover these rules and unparseable tokens.
Delegation decision parsing
Layer / File(s)
Summary
Decision parsing and regression tests evals/delegation-judgment/delegation_eval.py, evals/delegation-judgment/test_scorer.py
Mock decision extraction matches complete decision tokens at line starts and uses the last matching line. Tests cover glued tokens and multiple decision lines.
Approval revocation validation
Layer / File(s)
Summary
Revocation validation and tests evals/dispatch-regression-20case/approval_gate.py, evals/dispatch-regression-20case/tests/test_gate.py
The evaluator blocks missing or non-boolean revoked values. Tests verify that these cases produce no dispatches or dispatch events.
S3 public-access policy validation
Layer / File(s)
Summary
Public-access checks and tests specs/odin-tasks/local-iac-least-privilege-guardrail-validation/starter/policy/validate.py, specs/odin-tasks/local-iac-least-privilege-guardrail-validation/starter/tests/test_policy.py
Validation requires all four public-access settings to be exactly True. Tests cover missing settings and a string value.
Kiro relay documentation
Layer / File(s)
Summary
Relay description and hash references docs/operations/kiro-credit-failover.md, specs/kiro-credit-failover/design.md, specs/kiro-credit-failover/requirements.md
The operations guide identifies the deployed Telegram relay Worker as the live intake. The design and requirements documents state that the relay content hash remains in the private record.
Venture validation references
Layer / File(s)
Summary
Role-based scenario and trigger references specs/venture-validation-automation/design.md, specs/venture-validation-automation/requirements.md, specs/venture-validation-automation/tasks.md
The documents replace numeric identifiers with role-based references to the Venture Radar research owner, the read-only trigger, and the ODIN-only scenarios.

KyPython/ai-governance#1: Introduced the reusable governance workflow and its verdict gate, which this pull request updates.
Merge Risk: High · up to 86327
The shared governance gate's verdict script contains a shell syntax error, so the gate cannot produce a pass or fail verdict for any caller. This PR's own quality and policy checks also fail. The scorers can still accept an ambiguous action line as approved behavior, and the published scorecard can report pass while the gate fails. Fix the verdict script, refresh the recorded document hash, separate the spec edits, and tighten action-token parsing before merging.

Update the verdict predicate before defaulting the prerequisite to empty
For callers that use in-workflow quality and omit prerequisite-result, this new default prevents a passing verdict even when every required job succeeds. The existing inner condition at .github/workflows/governance.yml:1136 still requires PREREQUISITE_RESULT to equal success, so the newly computed prerequisite_ok does not actually permit the documented empty value. This affects the repository's own .github/workflows/self-test.yml, which omits the input. Replace the old predicate with the new prerequisite logic while retaining the security-job check.

Refresh the governing-document hash with this documentation change
Changing this document without updating .quality/system.json makes both the standalone system-quality workflow and the self-test's opted-in quality action reject the revision before running their checks. The definition still records the previous document's SHA-256, and scripts/system_quality.py:138 verifies it against the current contents. Running the declared runner reproduces governing requirements changed: docs/quality/system-evidence.md. Update the recorded hash alongside the document.

Separate the spec edits from the code changes
This commit changes files under specs/ alongside executable eval code and tests, violating AGENTS.md's explicit prohibition on coders editing spec paths in a code PR. The policy workflow also rejects this exact combination with this PR changes code AND spec files, independently of whether the edits only redact identifiers; its role gate additionally restricts spec authorship to Kiro or Ky. Move the spec-path changes into a separately authorized spec-only change rather than bundling them with these fixes.

Include prerequisite failure in the scorecard verdict.
When run-quality is false and the caller omits prerequisite-result, this condition fails the gate. The earlier scorecard step sees a skipped quality job as acceptable and does not inspect the prerequisite. It therefore publishes "verdict":"pass" and zero failed checks for a failed gate. Make the scorecard use the same prerequisite rule as the verdict step. (raw.githubusercontent.com)

Replace the old verdict condition with the new one.
The if at Line 1133 has no closing fi. Bash rejects the verdict script before it writes verdict to GITHUB_OUTPUT. This explains the CI errors that report a missing fixture output file. Adding fi alone is insufficient: the old condition at Line 1136 still requires PREREQUISITE_RESULT=success, so it rejects the new empty default. Remove the old condition and include SECURITY=success in the final condition. (raw.githubusercontent.com)

Refresh the governing-document hash.
.quality/system.json records a SHA-256 for docs/quality/system-evidence.md. These edits make that hash stale, so system_quality.py run fails with governing requirements changed before running the checks. Update the recorded hash in the same change. (raw.githubusercontent.com)

Reject action names followed by punctuation.
For ACTION: ASK/SEND, \b matches ASK before /. The scorer then returns C without checking an unapproved dispatch. Treat an action name as complete only when whitespace or the end of the line follows it. Apply the same correction in approval_eval.v1.py and approval_eval.v2.py, and add a regression case for the ambiguous line. The PR objective requires a whole action token.

Keep the decision declaration on one line.
The \s* after DECISION: also matches newlines. Therefore, DECISION:\nDELEGATE is parsed as a valid decision instead of returning None. Restrict this whitespace to spaces and tabs, and add a regression test for the split form. (raw.githubusercontent.com)

Move the spec edits into a separate spec-only PR.
This PR changes specs/kiro-credit-failover/design.md and specs/kiro-credit-failover/requirements.md while also changing code paths. The governance self-test therefore fails the spec gate. The role gate also rejects the current coder identity for spec changes.
Move both spec edits to a separate spec-only PR authored by Kiro or Ky. Merge that PR first. Keep only the implementation and documentation changes in this PR, then reference the merged spec as required by the gate. (github.com)

Review comment at @specs/venture-validation-automation/tasks.md at line 5:
Update the Research/fact-check gate task to require a private, stable mapping or
receipt that resolves the existing Venture Radar research owner and read-only
biweekly trigger to their exact Make resources, while excluding both ODIN-only
scenarios; do not include live resource identifiers.
